This pre-enrolment form is a necessity for students enrolling in a training course who are under 18 years of age. A parent or guardian will need to sign the medical disclosure on the enrolling students behalf.

To fill the forms out, insert your curser type. Hit the tab key to move to the next field. When you have completed the form print it out and sign the appropriate places.

Once the form is completed and signed, you can bring it with you to the training, mail or fax it to us.
